FAQS

What will I need? How can I order? Who can get my certificate?

All can be answered in our frequently asked questions page for information pertaining to birth certificates, death certificates, amendments, and affidavits.

  • Per state requirement, this office does NOT accept the temporary print out the BMV issues as an ID.

Protect Against Online Theft

There are many fraudulent websites claiming to order and send your birth certificate! We always recommend contacting the Vital Record Division of your birth county for online ordering before you place any order. Do not take it upon yourself to trust any website without clear instruction from local or state Vital Records.
